Schedule a CCAP Appointment

ATTENTION!

All CCAP visitors must show a Photo I.D. to security to receive any Walk-In service (including Walk-In Express).

Any visitors without a Photo I.D. or unwilling to comply will be refused service.

We are now offering Limited Walk-in Services!

Effective July 5th we are offering limited in-person walk-in services at our Damen and Chatham locations by appointment only. Schedule your 30-minute appointment here.

Changes to our Locations and Hours:

  • Damen and Chatham locations are open Monday, Tuesday, Thursday, and Friday from 8:30 a.m.-3:30 p.m.
  • Our Homewood and Lawrence locations are not currently open to the public.

Things to know:

  • You must have a scheduled appointment to receive a consultation.
    • Visitors without a scheduled appointment will not be serviced.
  • Phone appointments are on Wednesdays only.
  • Masks are required to receive service, even if you are vaccinated.
  • Appointments are scheduled for 30 minutes. Childcare providers can schedule back-to-back 30-minute appointments, if needed.
  • Visitors can receive and drop off CCAP forms without an appointment.
  • Please arrive on time for your consultation appointment.
    • If you arrive late, we cannot extend your consultation past your scheduled appointment time. You may have to schedule a new appointment.
  • Late arrivals of 15 minutes or more run the risk of losing their appointments. We may not be able to accommodate late arrivals as it may affect other customers that have appointments that day.
  • Providers should avoid booking more than 2 appointments at a time. When multiple appointments are made and not utilized, it takes an available time slot away from a customer.
    • If more than 2 appointments are made, we may cancel the additional appointments to accommodate the needs of our other customers.

CCAP phone appointments are available for parents and care and education providers. Phone appointments are available only on Wednesdays from 8:30 a.m. – 3 p.m.

Schedule a CCAP phone appointment when:

Parents

  • You need to submit your documentation and get a receipt via email;
  • You’re just getting started with CCAP – we can help you with the application and provide you with support and guidance on the required paperwork; or
  • You would like a scheduled time – no waiting on hold!

Care and Education Providers

  • You need updates on multiple cases;
  • Payment inquires for multiple families;
  • Submit forms on behalf of your families; or
  • You would like a scheduled time – no waiting on hold!

How it Works

  • When scheduling an appointment, you will be required to provide a phone number. When it is time for your appointment, a Family Resources Consultant will call you from a private number.
  • If you do not pick up, the Consultant will leave a voice message. They are not able to provide a call back number.
  • After five minutes, a second call attempt will be made. If there is no answer, you will need to schedule a new appointment.

Be Prepared for Your Phone Appointment

Before your scheduled phone appointment, please have the following ready:

  1. Your full name and date of birth
  2. The parent’s 15-digit case number(s) and their date(s) of birth
  3. The childcare provider’s 15-digit ID number, if applicable.

CCAP Phone Appointment Scheduler

Phone calls are scheduled for 30 minutes. An IAFC representative will call from a private number at your scheduled appointment time. We will make 2 attempts to reach you. If we are unable to reach you after 2 calls, your appointment will be cancelled, and you will have to reschedule.
